Cancellation Policy

Healthcare Science Academy CDPH-Approved Continuing Education Provider Provider #: __________ CEU Registration & Refund Policy 1. Registration Payment reserves a seat in the selected in-person CEU course. Due to limited seating, all course fees are final. 2. No Refunds All CEU course payments are nonrefundable. No refunds or credits will be given for cancellations, no-shows, late arrivals, early departures, failure to complete the course, missing required ID, removal for disruptive behavior, or failure to meet course requirements. 3. One-Time Reschedule One reschedule may be requested in writing at least 2 business days before the course date. Reschedules are subject to seat availability, must be completed within 90 days, and any price difference must be paid. Only one reschedule is allowed. Missing the rescheduled course forfeits all payments. Requests made less than 2 business days before class are generally not eligible unless approved in writing. Business days are Monday-Friday, excluding holidays and school closures. 4. Transfers Registrations may not be transferred to another person without prior written approval. Approved substitutes must meet all course requirements. 5. Attendance Participants must arrive on time, attend the entire course, and complete all required activities. Late arrivals or early departures may result in denial of entry or loss of CEU credit. CEUs are awarded only for completed instructional hours that meet CDPH requirements. 6. Course Changes Healthcare Science Academy may cancel or reschedule a course due to low enrollment, instructor availability, emergencies, or other circumstances beyond its control. If no reasonable replacement date is offered, participants may transfer to another course or receive a full refund. The Academy is not responsible for travel, lodging, lost wages, or other incidental expenses. 7. CEU Certificates Certificates are issued only after identity verification, full attendance, completion of required activities, signed attendance records, and meeting all CDPH/course requirements. 8. Acknowledgment By registering, participants acknowledge they have read and agree to this policy, understand that payments are nonrefundable, only one reschedule is allowed with proper notice, and CEU credit depends on verified attendance and course completion.
